現代人常喜愛佩戴耳機,無論聽歌、看戲甚至玩遊戲,雙耳都離不開耳機,同時,長期佩戴藍牙也是當下生意人常做的事,將藍牙掛在耳朵,方便隨時接聽來電,持續下去容易加劇耳朵損壞,尤其是神經線的損壞。 長期佩戴耳機容易引起神經線損壞,而神經線一旦損壞即無法修復,其也不會自愈,且必須長期佩戴助聽器幫助聽覺。 耳鼻喉專科醫生鄭應平舉例,如果一個人一星期連續5天使用耳機長達8小時,聲量持續在90分貝或以上,到了5年至10年的時間,他的耳朵神經線就會被損壞。 “一般人說話的聲量為30至50分貝,說悄悄話為20至30分貝,90分貝如爆炸聲或吶喊聲,都屬於危險的高分貝,容易損壞一個人耳朵的神經線。” 同時,根據研究報告,如果一個人長期使用一邊耳朵聽電話、佩戴藍牙,則常使用的那一邊將比少使用的那一邊耳朵更容易“受傷”,失去高頻率聽覺的功能更高。 他強調,耳鳴是耳朵已受損的徵兆,它不是一種疾病,而是正在告訴你:你的耳朵有問題了! _________________________ 耳機如何傷害耳朵: 長期使用耳機不但加劇耳朵神經線損傷,也容易引起外耳發炎,尤其是不干淨的耳機容易導致細菌感染,因此,注重耳機的衛生整潔是很重要的。 除了一般耳機,市面上有一種耳機特長,讓使用者直塞入耳朵,聲音傳播更清晰,聽者聽得更清楚。 “特長的耳機加劇刺破耳膜的風險,而且耳機完全塞入耳朵,如果音量過大,將引起過分的震動,容易導致耳膜破裂。” 鄭應平鼓勵民眾慎選耳機,而且應避免長期佩戴,也盡量維持音量在30至50分貝,即人們一般說話的音調,如果是車里聽歌,則可將音量數字調在10至11最佳,而18至20則屬於過分。 “一個人佩戴耳機,而坐在旁邊的人可以聽到耳機所傳過來的聲音,或者耳機音量太大聲,別人叫他聽不到時,則有關音量已超過50分貝,長期使用會對耳朵帶來損傷。” _________________________ 耳朵神經線受損能自愈嗎? 鄭應平表示,耳朵神經線受損不能自愈,也不能通過修復來解決,除非神經線血管未完全阻塞,否則一旦神經線損壞是無法修復的。 聲音從外耳輸入內耳的過程會經過耳膜,震動聽小骨(錘骨、砧骨及鐙骨)進入耳蝸,耳蝸毛細胞被震,把訊息傳給神經線,神經線傳導聲音連接至腦部,讓人由此接收並辨別外在的聲音。 “負責傳導聲音的神經線一旦損壞,是不能自動修復的,而這個人必須長期佩戴助聽器,否則將無法接收外在的聲音。” 鄭應平指出,聲音從外面傳入腦部的時間很快,除非耳朵內部出現阻塞,無法聽見或聽到很小聲,否則聲音傳播進入腦部是非常迅速且直接的。 “有時候聽到聲音,但聽不明白,不排除是耳朵神經線或哪里出狀況導致,因此,人家說話聽不明白,不一定是智商問題,而有可能是因為耳朵內部構造出現損傷的徵兆。” _________________________ “掏耳朵掏出癌症”? 鄭應平強調,掏耳朵只會傷及耳朵,不會掏出癌症,也沒有這樣的說法。雖然如此,他仍不鼓勵人們使用掏耳棒或棉花棒掏耳朵。 耳垢的學名為耵聹,也就是俗稱的耳屎。它的組成主要分為兩個部分,一是皮膚死掉的角質細胞自然脫落,二是外耳道腺體的分泌物與油脂。當兩者結合一起後,便成為大家所知的略帶黃色的“耳垢”,是一種新陳代謝的過程。 鄭應平透露,耳朵癢,或許是耳垢太多或是耳朵皮膚太乾燥。普遍上,耳朵皮膚乾燥所引起的癢,可滴上橄欖油,以滋潤乾燥的皮膚即可。千萬不要掏耳,以免越掏越癢。 他稱,大部分人的耳垢是不需要清洗的,因為外耳道有神奇的“自動清潔”功能。 “耳朵里死去的舊角質會逐漸被‘推’出來,所以我們不需要多此一舉掏耳,否則可能會因為掏耳不當,導致耳道受傷感染或造成耳膜的傷害。” 他表示,棉花棒的功用應為吸乾耳廓的水,而不是清耳垢。很多人習慣在沖涼後,用棉花棒掏耳垢,但是此舉卻容易將耳垢推得更深,造成耳垢堆積成硬塊、阻塞耳道。當耳垢越擠越深入,就會導致耳朵塞住,嚴重的還會遭細菌感染,進而發炎。 鄭應平說,若使用棉花棒掏耳,棉花棒頭不小心卡在耳道里,後果更不可言喻。嚴重的還可能因為使用力度太大,刮傷耳道或戳破耳膜,造成外耳道感染發炎或耳膜的傷害,千萬不可不慎!因此,不鼓勵人們自行使用掏耳棒或棉花棒。 他說,雖然一般人不需要特別清理耳垢,但有些人或因天生耳垢多、長期阻塞等因素,需要就醫請醫生清理耳垢。 耳垢阻塞耳道多半是因為時間過久,耳垢過多並已硬化。他建議,若有耳塞情況出現,應前往就醫,醫生會使用軟化耳垢的溶劑,如耳滴劑、嬰兒油或橄欖油等,使耳垢軟化後,再使用儀器把耳垢吸出來。 鄭應平再次提醒,對大部份人而言,耳垢是一種不需自行清理的東西,反之會自然把耳垢“掃”出耳外。至於少數人容易發生耳垢阻塞或其他問題的人,則必須定期接受耳鼻喉科醫生的檢查。 _________________________ 聽力下降年輕化? 截至目前,還未有數據證明砂州人的聽力下降有年輕化趨勢,若和過去十年比較,砂州人的聽力問題保持在平穩狀況。 鄭應平指出,人們的聽力下降與否和工作及住家環境有直接關係,若一人長期在吵雜的工廠工作,每天和機器為伍,或羅里駕駛者,長期接收引擎的吵雜聲,則容易加劇聽力下降的風險。 一般上,許多在工廠或機場工作的人員,都受到有關當局嚴厲監督,公司需定期為職員提供聽力檢測,確保在高風險環境工作的職員,聽力獲得保障,避免延伸聽力損傷的問題發生。 _________________________ 初生兒接受醫院聽力檢測: 截至目前,砂州共有3間中央醫院分別是古晉、美里及詩巫陸續為在醫院出生的嬰孩,提供聽力檢測,提早發掘孩子的聽力是否出現障礙,並在發現問題時,讓孩子及早就醫,避免誤了時間。 鄭應平表示,一旦醫生發現初生兒有聽力障礙,醫院將安排孩子三個月後重返醫院接受掃描檢測,根據個案的狀況提供進一步治療。 接受治療的最佳時期以4歲前最佳,那時的腦部較容易接收聲音,接受治療的患者當中,有70%至80%患者可像正常人一樣生活,若在4歲之後才接受治療,則療愈的巴仙率將有所降低。 截至目前,助聽器是改善聽力問題的最佳方法,如果助聽器無法讓患者聽到聲音,可用手術治療,重獲聽覺。 鄭應平鼓勵家長在孩子小時候,讓他們多聆聽不同音樂,接受不同的聲音和頻率,惟音量需適量。 “聆聽不同的音樂,讓他們的腦部接收不同的聲音和頻率,可幫助腦部發育,促進健康成長,而鋼琴聲是不錯的選擇。” _________________________ 小常識: 問:除了耳垂部位,在其他耳朵部位穿耳洞是否會對耳朵造成傷害? 答:建議只穿(耳洞)耳垂部位,因為其他部位有軟骨,若弄傷軟骨會導致細菌感染,嚴重時耳朵甚至會生瘢痕(肉芽)。 問:扭耳朵是否會造成耳朵傷害? 答:用力過度,將會導致耳朵出血或積血,嚴重之時甚至需要動手術“放”血。 問:打耳光會導致什麼樣的傷害? 答:除了炸彈的爆炸聲,打一個耳光也會把耳膜打破。一個耳光就已高達70至90分貝,當耳膜受到突來的巨大壓力及振動而破損時,耳朵便會出血,甚至出現聽不見的狀況。嚴重時,耳內的聽小骨甚至會因此而脫節。 問:飛機升降時,為何會出現耳痛的狀況? 答:中耳存有少量空氣以平衡耳內與耳外的氣候壓,當飛機起飛或降落,耳壓與外界環境便有所不同,當輸軟管阻塞後,就會產生耳痛。吃糖果或打哈欠都有助“打通”阻塞的輸軟管。… Read more »

有時候,當你感到有眩暈、嘔吐的症狀時,身邊的人就會問:“你是不是耳水不平衡?”究竟耳水不平衡是怎麼一回事?而怎樣的感覺才能被稱為“眩暈”呢? 據了解,眩,是指目眩,眼花繚亂;暈,是指頭暈,頭部轉動不定。兩者若同時出現,便成為“眩暈”。 其實,我們的耳朵不只是負責聽覺的器官,它還負責身體的平衡,而引致眩暈的成因,有部分是由內耳問題所產生的。 耳鼻喉專科醫生鄭應平表示,眩暈是常見的一種疾病,且病因也有很多種。有時候,當你有眩暈的症狀並不一定是耳水不平衡所引起,或許是其他因素所引起的。 “因此,當你出現眩暈伴隨嘔吐現象時,雖可以掛耳鼻喉科瞭解內耳平衡是否有問題,但如果你覺得頭暈、嘔吐及身體半邊沒力(恐有中風之虞),又或是若感到頭暈及心跳異常加速(恐有心臟病問題),病患應前去緊急部門做檢查,先排除重症再說。” 所謂的耳水不平衡,可被稱為良性陣發型姿勢性眩暈(BPPV),它其實是一種內耳造成的問題。當你的頭部移動到某特定位置時會發生眩暈,這可能使你覺得頭昏眼花、失去平衡及感到噁心。 那又是什麼引起良性陣發型姿勢性眩暈?鄭應平透露,耳石是控制人身體平衡的重要器官,正常的耳石應在三個半規管中。 “當在我們頭部的移動下,耳石在球囊橢圓囊(Utricle)中,一旦耳石落下來並跑到三個半規管(Semicircular canals),就會發生良性陣發型姿勢性眩暈。” 他指出,耳石脫落是最常見的眩暈原因之一,當患者在移動頭部或改變姿勢時會感到眩暈,但有許多病人的病因是不明的。俗稱的耳水不平衡患者,較多為中年人,男女機率同等,且也沒有所謂的職業限制。 _________________________ Epley手法: 是最常見的一種周圍性眩暈,病變多在後半規管。以Epley手法可減輕或緩解眩暈不適症狀。(配合power point Epley Maneuver的4張圖) A. 患者坐於治療台上,在治療者幫助下迅速取仰臥懸頭位,並向患側(Diseased side)扭轉45°。 B. 頭逐漸轉正,然後繼續向健側(Healthy side)偏45°。 C. 將患者頭部連同身體向健側翻轉,使其側臥於治療台上,頭部偏離仰臥位達135°。 D. 坐起,下巴往下傾20度。 _________________________ 談眩暈: 鄭應平表示,眩暈是常見的一種疾病,並可分為兩類,即真性眩暈(True vertigo)及假性眩暈(Non rotatory vertigo)。 他說,真性眩暈(旋轉性)的原因有二,即中樞神經系統(CNS)及耳科所引起的眩暈。中樞神經系統所引起的眩暈,如有視力改變、麻木/輕癱、感覺異常(Altered sensorium)、說話異常(Altered speech)、吞嚥困難及小腦徵候(Cerebellar signs)。 “至於耳科所引起的眩暈則包括發作性(Episodic)、噁心嘔吐、劇烈移動及眼球震顫症(Nystagmus)等。” 他稱,假性眩暈(非旋轉性)則由腦血管痙攣(CVS)如心律失常、貧血或體位性低血壓(Orthostatic hypotension);藥物因素,如降高血壓藥、抗癲癇藥、鎮靜劑/安眠藥;其他因素如代謝與精神科所引起。 _________________________ 鄭應平透露,耳科眩暈(Otological Vertigo)又可分為梅尼爾氏症(Meniere’s Disease)、良性陣發型姿勢性眩暈、前庭神經炎(Vestibular neuronitis)及復發性前庭病(Recurrent vestibulopathy)。 他解釋,梅尼爾氏症的眩暈症狀可持續數分鐘至數小時,症狀如聽力受損、耳鳴、耳壓、內淋巴水腫及水平性眼球震顫症(Horizontal nystagmus)。 “良性陣發型姿勢性眩暈則無聽覺問題,眩暈引起的症狀包括頭部位所引發的、半規管耳的結晶及旋轉性眼球震顫症(Rotatory nystagmus),眩暈狀態會持續數秒。” 他說,前庭神經炎的眩暈狀態可持續數天至數週,通常發作於上呼吸道感染(流行性感冒)過後,或是由前庭神經病毒感染及水平性眼球震顫症所引起的。 鄭應平續稱,復發性前庭病的眩暈症狀可持續數分鐘至數小時,通常是自發性的,而復發性前庭神經傳入阻滯及水平性眼球震顫症皆可引起。 _________________________ 耳朵疾病: 耳朵疾病分成外耳、中耳及內耳,意即耳朵內部不同部位損傷所引發的疾病。 鄭應平表示,外耳常見的疾病以外耳炎最普遍,如一個人頻密挖耳垢或水滲入耳朵,引發細菌感染而發炎。… Read more »
